P a t i e n t S b e n e f i t f r o M h u n t e r D o n C o u n t Y M e D i C a t i o n a C C e S S P r o g r a M The Hunterdon County Medication Access Program meets a critical com- munity need and has improved access for the county's uninsured and underinsured residents to prescription medications, thereby improving health and minimizing excessive and avoidable use of the emergency department. The program utilizes a nurse educator to coordinate services and provide residents with access to affordable prescription medications and health education. The program works with pharmaceutical company patient assistance programs to provide patients with medications at a lower cost or no cost at all. "This program has helped patients incorporate positive behavior changes into their lifestyle, provided medication and disease-specific informa- tion and coordinated prescription refills with the treating physician, patient and the Pharmaceutical Company," explained Karen Kallens, RN Nurse Educator for HCMAP. Through HCMAP, Hunterdon Medical Cen- ter has collaborated with physician practices, Hunterdon County Department of Health, offices of senior services, Institute for Medication Access and Compliance, local congregations and social and human service agencies to refer patients and support the activities of this program. Since the program began in 2004, HCMAP has helped over 3500 people receive the medications needed to stay healthy. Last year, the average wholesale price of medications requested was over $500,000 and the program has saved residents over $5 million in medication costs. HCMAP works closely with patients and their physicians to ensure they receive the maximum amount of assistance.
